Neuropsychological Evaluations

Shanna Cooper, PhD, ABPP-CN, is a licensed clinical psychologist with expertise, including board certification, in clinical neuropsychology. She conducts specialized, comprehensive forensic, clinical, and FAA neuropsychological evaluations. Dr. Cooper leverages science-based, ethical, and professional procedures and systematic approaches in rendering her opinions.
Referrals:
Dr. Cooper accepts referrals from attorneys, agencies, and FAA candidates. Dr. Cooper does not work directly with any insurance companies for these types of evaluations.
Payment:

Forensic evaluations are billed via the referring attorney. A fee schedule is available upon request.

FAA evaluations are completed on a full-payment, self-pay at time-of-service basis.

Agency evaluations are billed through the referring source.
For FAA evals:
You must have a letter from the FAA indicating a neuropsych evaluation is needed before I can schedule you. From there, I must have your FAA medical file and any other medical or personal records that you feel would help me form an objective opinion of your case. Please send me these materials as soon as possible/at least 2 weeks ahead of your appointment. I will issue my report to the FAA within 30 days of the evaluation or medical records receipt, whichever is later. ​A team of doctors at the FAA will make the final determination regarding your flight status based on your full history, including but not limited to my report.
